Customers or resellers who receive a product that is faulty upon initial installation should follow these steps:
- Complete the required Poly DOA Certification Form (Form PGS3.1), which can often be obtained from the HP Support website (click here) or via email from a support representative.
- Email the completed form to doacertificate@poly.com.
- Include a copy of the completed form with the physical shipment of the faulty product back to Poly.
Submitting this form is a required part of the RMA (Return Material Authorisation) process to get an advance replacement for the defective unit.
